***元/人/年（二）保险责任介绍 1 、意外身故保障在保险期间内,若被保险人遭受意外伤害事故，并因本次意外伤害直接导致被保险人在该意外伤害事故发生之日起 180 日内身故，***按该被保险人对应的保险金额***故保险金。 2 、意外残疾保障在保险期间内,若被保险人遭受意外伤害事故，并因本次意外伤害直接导致被保险人在该意外伤害事故发生之日起 180 、定期寿险保障在保险期间内，若被保险人因疾病导致身故，***按该被保险人对应的保险金额***险金。 5 、疾病住院保障在保险期间内，若被保险人因疾病（不含严重既往症）住院要直接用于治疗的符合基本医疗保险管理规定的费用，***按社保统筹结算后社保范围内用药 100% 赔付。（三）服务要求 1 、承保服务（1）采用纸质和电脑数据相结合的建档方式，按区域建立健全客户档案资料库，供相关部门参阅。（2）指定专门的客服人员，接受各方咨询，解释相关保险内容。（3）定期走访项目专员，听取相关意见和建议，不断提高服务水平。（4）如有需要每季度按时向***区退役军人事务局相关部门汇报上季度的理赔情况。 2 、人员配备（1）***对本采购项目须设立专项小组。其中项目经理 1 名，负责按照本次保险的有关要求为各被保险人提供承保、理赔及其他各类保险服务，包括必要的组织与协调工作；并指定专人负责该项目的日常服务工作，客户服务专员：至少配备 2 名，其中有 1名应为项目专职承保人员， 1名应为项目专职理赔人员，协助项目经理提供保险服务（提供专项小组名单）。（2）根据相应文件内容，配合投保人完成核保与出单事宜，并在收到投保单和保费五个工作日内将保单正本和保费发票派专人送达投保人。 3 、理赔服务（1）保险服务基础标准： ①统一实行24小时接报案； ②统一实行24小时查勘； ③统一实行24小时救援； ④统一实行8小时全年无休理赔工作； ⑤统一实行24小时投诉服务； ⑥设立24小时保险服务专线电话，电脑录音系统和查勘调度系统保证畅通无误，及时准确。全年 365 天随时接受被保险人的出险报案，***本项目客户服务专员。（2）设立专人专门窗口负责受理赔款案件，实行一站式服务，确保理赔、财务、信息技术等环节的高效畅通。（3）接到报案后应详细的告知被保险人注意事项、承保范围、保障内容及理赔时需要准备的材料等，不得欺骗、误导被保险人。（4）配合被保险人了解、熟悉理赔流程，指定专人上门收取索赔单证，并在约定的时间内上门收取，不得以任何理由拒绝上门服务。（5）***上门收取理赔资料时，若认为有关证明或材料不完整，立刻以书面或口头方式通知被保险人需补充提供的有关证明或材料。***在接到索赔资料后 1 个工作日内未给予有关审核意见，则视为认可索赔资料的完整性。（6）在收到理赔材料后，简化理赔手续，应在规定时间内告知被保险人赔偿金额***或不予赔偿的理由，超过规定时间的，视为同意赔偿。（7）接到被保险人报案后，对于属于保险责任的事故，资料齐全后 10 工作日内确定赔偿金额***对于不属于保险责任的，同意在接到被保险人赔偿请求后 5 个工作日内向其发出《拒赔通知书》。（9）对于不属于保险责任的，***在接到被保险人赔偿请求后没有按规定时间向其发出《拒赔通知书》，则按保险责任处理。（10）每半年度向***区退役军人事务局相关部门提交《保险服务报告》，年终向***区退役军人事务局相关部门提交《年度服务综合报告》。（四）根据《政府采购促进中小企业发展管理办法》（财库〔2020〕 46 号）及《中小企业划型标准规定》（工信部联企业〔 2011〕 300 号），本采购标的对应的中小企业划分标准所属行业为：其他未列明行业。合同履行期限：2年，具体保险期间以保单所载为准。本项目接受多个供应商组成联合体参加磋商，若以联合体响应，联合体最多不超过 2 家。二、申请人的资格要求： 1 、满足《中华人民共和国政府采购法》第二十二条规定：（ 1 ）具有独立承担民事责任的能力。（ 2 ）具有良好的商业信誉和健全的财务会计制度。（ 3 ）具有履行合同所必需的设备和专业技术能力。（ 4 ）有依法缴纳税收和社会保障资金的良好记录。（ 5 ）参加政府采购活动前三年内，在经营活动中没有重大违法记录。（ 6 ）法律、行政法规规定的其他条件。 2 、落实政府采购政策需满足的资格要求：无。 3 、本项目的特定资格要求：（1）磋商供应商具有中国保监会核发的《保险许可证》。（2） 证书的供应商进行网上注册及资料填报后，携带CA证书至苏州 CA 服务网点开通***区区级政府采购项目网上报名功能模块即可。 2 、登录报名。供应商进入“***市公共资源交易平台”，“政府采购（区级网上报名）”系统，插入 CA 证书登录后选择具体项目并点击“报名”按钮报名。报名后，系统会显示报名确认单，供应商须以供参与项目使用。供应商报名成功后不可撤销报名。 3 、依法获取采购文件。供应商进行网上报名后在报名系统中自行下载采购文件，报名日期视同为依法获取采购文件日期。注：供应商、保留“网上报名确认单”，质疑时与质疑函一并提交。未依照采购公告要求实行网上报名的供应商，视为未参与该项政府采购活动，不具备对该政府采购项目提出质疑的法定权利，但因供应商资格条件或报名时间设定不符合有关法律法规规定等原因使供应商权益受损的除外。 4
